Allen School Feedback Tool

The Allen School Feedback tool allows you to provide constructive feedback and report problems semi-anonymously. Your name will not be included in reports that this system generates and sends to the appropriate Allen School faculty and staff. Naturally, they will not be able to reply to you.

Anyone in a CSE course can provide feedback about a specific course. Members of the Allen School community can login with CSE credentials to provide general School feedback, as well as to provide feedback about courses they are taking or have taken recently. This service does require you to login with your UW or CSE credentials in order to guard against unwarranted spam or abuse. In general, your identity will not be revealed to the recipient of the feedback unless you disclose as much in your comments. However, we may use the tool’s authentication logs to identify who submitted feedback in the following circumstances:

  1. Repeated use of the tool to harass Allen School faculty and staff, as determined by the Allen School Director or Vice Director, in order to make a referral to the appropriate campus body to adjudicate apparent misconduct.
  2. If required to do so as part of an investigation into alleged misconduct.

To report discrimination, harassment, sexual misconduct, or related retaliation, including violations of Executive Order 81, the proper process is to make a Civil Rights and Title IX Report. To learn about support options, reporting and what happens after a report is made, please visit the UW Civil Rights Compliance Office website or contact their team at civilrights@uw.edu. If you report discrimination, harassment, sexual misconduct, or related retaliation using this feedback form, we will have to share it with the Civil Rights Compliance Office. We will not include your identity when sharing your report but, as noted above, may be required to disclose it as part of any investigations that follow.

This form is not an appropriate channel for supervisors to report possible violations of Executive Order 81.
